Breakthrough Gel for Arthritis Treatment

Arthritis is a crippling disease that affects millions of people worldwide. Some suffer only mild symptoms but at its worst it is truly debilitating and extremely painful with treatment bringing patchy relief for many. Both rheumatoid arthritis and osteoarthritis are characterized by often debilitating pain in the joints and a new method of relief could be at hand.

Researchers at Brigham and Women’s Hospital in the USA) report on an injectable gel that could spell the future for treating these diseases and others. Among its advantages, the gel could allow the targeted release of medicine at an affected joint, and could dispense that medicine on demand in response to enzymes associated with arthritic flare-ups. Arthritis is a good example of a disease that attacks specific parts of the body. Conventional treatments for it, however, largely involve drugs taken orally. Not only do these take a while (often weeks) to exert their effects, they can have additional side effects. That is because the drug is dispersed throughout the body, not just at the affected joint. Further, high concentrations of the drug are necessary to deliver enough to the affected joint, which runs the risk of toxicity

Nor could this new development be limited solely to arthritis but researchers believe could be useful for multiple medical applications including the localized treatment of cancer, ocular disease, and cardiovascular disease.

Jeffrey Karp, leader of the research and co-director of the Center for Regenerative Therapeutics, recently won the coveted SFB Young Investigator Award for this work. It is currently of course possible to inject a drug into the target area, but it won’t last long–only minutes to hours–because it is removed by the body’s highly efficient lymphatic system. There are also available implantable drug-delivery devices but these have drawbacks: most are made of stiff materials that in a joint can rub and cause inflammation on their own and they generally release medicine continuously–even when it’s not needed. Arthritis, for example, occurs in cycles characterized by flare-ups then remission.

A series of experiments confirmed that the gel can release encapsulated agents in an on-demand manner and although the team has yet to test this in humans, they did find that in mice the gelremained stable for at least two months. Further, the gel withstood wear and tear representative of conditions in a moving joint.

Additional tests in mice are underway and a patent has been applied for so they can start research on human subjects.

Painkillers, Deafness and Arthritis

Not your usual sort of headline, but although most of us take painkillers occasionally, or regularly for chronic conditions, there are some well-established downsides such as gastric problems.

However, a new in the American Journal of Medicine alerted me to something I didn’t know, and that is that regular use of painkillers is linked to an increased risk of hearing loss. Even small amounts, such as for example taking paracetamol at least twice a week, can put you at risk of a massive 99 per cent increase in the chance of you having mild to severe deafness before the age of 50. If you regularly take ibuprofen or aspirin then that is not as high a risk, but is still a staggering 61 per cent increase over those who never take painkillers.

So that is something to think about, and particularly for those who take painkillers regularly for chronic conditions such as arthritis. If you suffer in this way you are more than likely to be prescribed non-steroidal-anti-inflammatory drugs (NSAIDs) by your doctor, and these include aspirin and ibuprofen.

If you have been taking such drugs in the belief that they will help with the pain, the evidence does not support it. One trial that looked at the long-term effects of NSAIDs versus placebo on pain showed “no significant effect of NSAIDs compared with placebo at one to four years” and over six years the British Medical Journal published a review of 23 studies that reinforced this view. They concluded that “NSAIDs can reduce short term pain in osteoarthritis of the knee slightly better than placebo, but the current analysis does not support long term use of NSAIDs for this condition. As serious adverse effects are associated with oral NSAIDs, only limited use can be recommended.”

All very well, but what can you do about the pain from arthritis? Natural alternatives such as white willow bark (the natural substance that aspirin was synthesised from) and many find acupuncture of great help in relieving long term and chronic pain. Personally I recommend you get hold of Patrick Holford’s excellent book ‘Say No To Arthritis’ for a thorough look at all the options from anti-inflammatory essential fats from oily fish to plant extracts such as boswellia, bromelain and ginger.

Flip Flops and Osteoarthritis

If you suffer from osteoarthritis in your knees then there may be no need to spend a lot of money on expensive specialist shoes. A study by Rush University Medical Center has found that flip-flops and sneakers with flexible soles are easier on the knees than clogs or even special walking shoes.

Osteoarthritis is the most common form of arthritis and a significant source of disability and impaired quality of life. A higher-than-normal load on the knees during walking is a hallmark of the disease, associated with both the severity of osteoarthritis and its progression.

Dr. Najia Shakoor, a rheumatologist at Rush and the primary author of the study, said “Traditionally, footwear has been engineered to provide maximum support and comfort for the foot, with little attention paid to the biomechanical effects on the rest of the leg but the shoes we wear have a substantial impact on the load on the knee joints, particularly when we walk.” They analyzed the gait of patients with symptoms of osteoarthritis while they walked barefoot and with four popular shoe types: Dansko clogs, which are often worn by those who have to be on their feet much of the day; stability shoes; Puma sneakers; and flip-flops.

The loads on the knee joints differed significantly depending on the footwear. For the clogs and stability shoes, the loads on the knee joints were up to 15 percent greater than with the flat walking shoes, flip-flops or barefoot walking. Knee loading was roughly the same whether the subject wore flips-flops or walked barefoot.

Several aspects of footwear affect the joint loading: heel height and stiffness of the sole being the most important. Earlier studies have shown that barefoot walking is associated with lower knee loads than walking with conventional footwear. Flip-flops and sneakers are flat, flexible and lightweight and seem to mimic the mechanics of walking with bare feet.

Clogs and stability shoes that are usually suggested to provide appropriate cushioning and support were shown to actually increase the loading on the knee joints, as opposed to shoes with less ‘support,’ flatter heels and more flexibility. But, bear in mind that flip flops could contribute to falls because of their loose-fitting design so maybe seek out some fancy sneakers instead.

Can your dentist help your arthritis?

If you are one of the half a million people in the UK who have rheumatoid arthritis, and I include myself among them, then going to the Dentist doesn’t usually suggest itself as a cure. However, some new studies published in the Journal of Periodontology show that there is definitely a link between a healthy mouth and a healthy body.

Research by the Case Western Reserve University School in Cleveland USA supports existing reports linking gum disease with arthritic pain and inflammation. Arthritis is definitely inflamed by toxins in the body and tooth decay and gum disease are both natural homes for toxins. They monitored 40 people with moderate or severe periodontitis and severe rheumatoid arthritis to see if dental treatment made any difference to their arthritis. If the subjects had toothache or painful teeth then extracting them seemed to have a positive impact on arthritic pain. It was made even better if the dental treatment was combined with taking anti-inflammatory drugs.

Gum disease no joke
A huge number of us will suffer gum disease at some point in our lives, with some estimates putting it as high as 19 in 20 people. There are significant potential risks as gum disease has been linked to not only arthritis, but heart disease, strokes, diabetes and premature births.

Celery and the brain

Researchers at the University of Illinois report that a plant compound found in abundance in celery and green peppers can disrupt a key component of the inflammatory response in the brain. This could be important news for the research on ageing, and on diseases such as Alzheimer’s and multiple sclerosis.

Inflammation plays a key role in many neurodegenerative diseases and also is implicated in the memory and behaviour problems that can arise as we get older. Inflammation is not always a bad thing; it is a critical part of the body’s immune response that in normal circumstances reduces injury and promotes healing, but when it goes wrong then it can lead to serious physical and mental problems.

The new study looked at luteolin, a plant flavonoid in celery and green peppers which is known to impede the inflammatory response in several types of cells outside the central nervous system. Herbalists have known about the cooling properties of celery for decades and prescribe it for arthritis and hot flushes, but now it seems scientists are also taking it seriously. Add celery and green peppers to your diet and you will whizzing through the crossword in record time. If you don’t like the taste of them – and I know some people who don’t – then if you have a juicer add it to your mix. I juice celery regularly with apples and carrot to boost my immune system and help with arthritis and even celery-haters love the taste of the juice.

Ayurvedic herb relieves osteoarthritis

It now seems that an Ayurvedic herb known as the ‘Indian Frankincense’ can dramatically improve the symptoms of the most common form of arthritis — osteoarthritis. No surprise to me as I have found all forms of herbal medicine to be effective, whether English, Chinese or Ayurvedic, but what is amazing is that treatment appears to provide relief within just one week.

I discovered this in the journal Arthritis Research & Therapy where scientists from the University of California at Davis published their research conclusions when they tested an extract dubbed AKBA (3-O-acetyl -11-keto-beta-boswellic acid) from the herb Boswellia serrata on 70 patients with osteoarthritis of the knee. Boswellia is the gummy resin of the boswellia tree, which is native to India, and used for centuries by Ayurvedic doctors as it contains anti-inflammatory terpenoids called boswellic acids. Boswellia is often included in arthritis remedies, and has been used for thousands of years by Ayurvedic practitioners.

The research subjects were suffering from pain, limited movement, stiffness and other symptoms of osteoarthritis, which most commonly affects weight- bearing joints like knees and hips, as well as the hands, wrists, feet and spine. Symptoms were relieved in about seven days in this study — which is the first to show that an enriched extract of Bowellia serrata be used as a successful treatment in humans. The researchers concluded that its anti-inflammatory properties can be an effective treatment for osteoarthritis of the knee and was shown to have no major adverse effects in osteoarthritis patients and so was safe for human consumption and even for long-term use.

Rose hips & arthritis

I have mentioned the benefits of rosehips before, and now a new study shows that it could be a more effective pain-reliever than standard drugs for people with arthritis, a new study suggests.

A powder form of the wild variety of rosehip, Rosa canina, is better at relieving pain among osteoarthritis patients than paracetamol and the nutritional supplement, glucosamine according to researchers from Frederiksberg Hospital in Copenhagen.

All three therapies were tested on a group of 300 patients, who tested each in turn for three months. Overall, the patients reported that the rosehip preparation was almost three times more effective than paracetamol, and 40 per cent better than glucosamine. Rosehip also didn’t come with the side effects associated with paracetamol, including constipation and drowsiness. The researchers concluded that rosehip is so effective because it also reduces the inflammation in the joints, which is characteristic of osteoarthritis.
